After discharge from hospitals, individuals experiencing homelessness often still need medical support and stability for their healing journey. Love Focus’s recuperative care program provide quality care to our neighbors experiencing homelessness, recognizing everyone deserved to have a dignified experience that is individualized in their path to health and home. Our recuperative care site is a homelike environment for healing and recovery. We partner with health plans to provide services to Members receiving Recuperative Care (Medical Respite), as well as providing services to Members receiving Short-Term Post-Hospitalization Housing Services in either Recuperative Care (Medical Respite) or non-Recuperative Care (Medical Respite) beds.

Medical Oversight [for Recuperative Care (Medical Respite) only]

  1. Coordination of medical care under oversight of Medical Director.
  2. Daily wellness assessments/welfare checks (medical, BH, SUD), including ongoing assessments of vital signs and acute symptoms.
  3. Medication reconciliation and monitoring.

Care Coordination/ Case Management/ Member Navigation

  1. Administration of comprehensive assessment of baseline Member health and development of individualized care plans [for Recuperative Care (Medical Respite) only]
  2. Behavioral health and substance use disorder services.
  3. Linkage to specialty care services, if needed [for Recuperative Care (Medical Respite) only].
  4. Benefit and entitlement acquisition.
  5. Housing navigation, including completion of VI-SPDAT or other housing assessment tool and entry into local Coordinated Entry System.
  6. Referral to community and social supports.
  7. Transportation.
  8. Connection to PCP/medical home [for Recuperative Care (Medical Respite) only].
  9. Discharge Planning.
  10. Bed, meals and full bathrooms.
  11. Security measures.

Community Health Worker (CHW) Services

Our CHW Services program aligns with Medi-Cal community health worker (CHW) services, pursuant to 42 CFR Section 440.130(c), as preventive services and on the written recommendation of a physician or other licensed practitioner of the healing arts within their scope of practice under state law.

CHW services may address issues that include, but are not limited to, the control and prevention of chronic conditions or infectious diseases; mental health conditions and substance use disorders; need for preventive services, perinatal health conditions; and reproductive health; environmental and climate-sensitive health issues; child health and development; oral health; aging; injury; domestic violence; and violence prevention.

Definition
Community health worker (CHW) services are preventive health services to prevent disease, disability, and other health conditions or their progression; to prolong life; and promote physical and mental health.

Our Community health workers may include individuals known by a variety of job titles, including promotores, community health representatives, navigators, and other non-licensed public health workers, including violence prevention professionals, with the qualifications.
